[https://jeffersonhealthplans.softheon.com/account/payments/locate-account](https://jeffersonhealthplans.softheon.com/account/payments/locate-account)  并输入您的个人信息来查找账户, 可以使用社安号后四位或者保险卡上的ID号码来查询
