How to Access Your Jefferson Health Insurance Portal , Change PCP, and Access Card

Your Health Trio Connect portal allows you to manage your health plan online. Follow the steps below to log in, recover your account, change your primary care provider, and download your ID card.

How to Register an Account

  1. Visit the Health Trio Connect portal:

Having Trouble Logging In?

If you receive an error message, it usually means you already have an account.

  • Go back to the login page.

  • Click “Forgot Username.”

  • Your username will be sent to the email address on file (the same as your Pennie Account).

  • Once you receive it, return to the portal.

  • Log in using your username and reset your password if prompted.

How to Change Your Primary Care Provider (PCP)

After logging in:

  1. Click on “My Providers.”

  2. Select “Update.”

  3. Choose a new Primary Care Provider and save your changes.

How to Get Your Insurance ID Card

To view or download your ID card:

  1. Click on “My Health Plan.”

  2. Select “ID Card Request.”

  3. Click “Print.”

    • You can save the card as a PDF and share it if needed.
